Volunteer Application- This application is the first step to becoming a volunteer with the SESP Program. Please download, complete & sign it and return it to the SESP Program at the address on page 4.

Change Report Form- This form is for all current volunteers. If there is a change in your status as a volunteer, or a change in the status of one of your students, please complete this form and submit it to our office.

SESP's Rights And Responsibilities - This is a list of the Volunteer SESP's Rights and Responsibilities. You will receive this form when you are appointed a student.

Student Referral Form - This is the 2 page form in which to refer a student to the SESP Program. All students must be in state custody and either have, or have been referred for Special Education.
